TeleHealthNP Terms of Service
Effective date: September 1, 2026
These Terms govern the website and services offered by TeleHealthNP, LLC, doing business as TeleHealthNP (“TeleHealthNP,” “we,” “us,” or “our”). Our contact details appear below. Please review these Terms before enrolling or making a purchase.
These Terms apply when presented to you and accepted through our applicable enrollment or purchase process. Your treatment also requires any separate clinical and telehealth consents. These Terms do not replace informed consent, our Notice of Privacy Practices, or rights provided by law. Any plan-specific terms must be presented before purchase and read together with these Terms.
1. Services and Eligibility
TeleHealthNP provides healthcare and related services through licensed clinicians. Services are available only where the treating clinician is legally authorized to practice and the service is appropriate for the patient. We currently provide care in Oregon, Washington, and Arizona. You must accurately disclose your physical location at each clinical encounter; residence in a service state alone does not establish eligibility when you are elsewhere.
Minimum patient age: 18. We may verify identity, age, location, and authority to act for someone else. Clinical eligibility depends on health history, current medications, the clinician's assessment, and applicable law.
Website access, a general inquiry, or payment alone does not guarantee acceptance for treatment, a prescription, or a particular medication. A clinician-patient relationship arises as provided by law and our clinical intake process. These Terms do not override duties arising when a clinician actually provides care or individualized medical advice.
2. Educational Content and Emergencies
Website articles, newsletters, social posts, and general product information are educational and are not individualized diagnosis or treatment. Do not change prescribed treatment based solely on website content. Ask your clinician about advice relevant to your circumstances.
TeleHealthNP does not provide emergency services. Website forms, email, text, and portal messages are not emergency-response channels. Call 911 or seek immediate local emergency care for an emergency.
3. Telehealth Care and Your Responsibilities
Telehealth has limitations, including technology problems and the inability to perform some physical examinations or tests remotely. Your clinician may require testing, an in-person visit, or referral to another provider. The separate telehealth consent describes the material risks, benefits, alternatives, and other information required for your care.
Provide accurate and complete information about medical history, allergies, medications, pregnancy or breastfeeding when relevant to care, and other circumstances affecting treatment. Tell your clinician about changes, adverse effects, and care received elsewhere. Follow the prescribed instructions and arrange recommended monitoring or follow-up. Contact the practice if instructions are unclear.
Use a private location and a reliable connection for appointments when possible. If a connection fails, we will reattempt the call or reschedule for a time when you can establish a reliable connection. These responsibilities do not reduce the clinician's professional obligations or your rights.
4. Prescriptions and Medication
Prescribing decisions are made independently by the treating clinician. Enrollment does not entitle you to a prescription, a dose increase, continued refills, or any named medication. Refills may require a visit, laboratory results, monitoring, or an updated assessment. Controlled-substance prescribing is subject to additional federal and state requirements; availability is not guaranteed.
Medication selection, dose, and duration are individualized. Amounts expressed in milliliters describe volume and are not interchangeable with milligram doses; follow the specific prescription and concentration instructions. Do not share medication or use another person's prescription.
Where a compounded medication is prescribed, the particular compounded product is not FDA-approved and has not undergone FDA review for safety, effectiveness, or quality. Clinical results for FDA-approved products should not be assumed to establish identical results for a compounded product. The clinician and dispensing pharmacy determine whether dispensing is lawful and appropriate. Separate medication counseling or consent may apply.
Weight change and other treatment outcomes vary. We do not guarantee a specific result, timeframe, lack of side effects, or treatment suitability.
5. Pharmacies, Shipping, and Orders
Depending on your plan, a prescription may be sent to a pharmacy you select or to a dispensing pharmacy arranging delivery. Dispensing, availability, substitutions, shipment conditions, and fulfillment remain subject to law and the pharmacy's requirements. A third-party pharmacy may require its own consent or terms.
Provide an accurate shipping address and review any delivery, storage, or temperature instructions. Contact 602-810-0868 promptly about missing, damaged, delayed, or temperature-compromised shipments. Do not use medication that appears unsafe until you receive appropriate guidance. This does not limit any legal remedy for an incorrect or defective order.
Medication Shipping, Returns, and Refunds: Standard medication shipping is included in the subscription price; no separate standard shipping charge applies. Medication delivery is available to eligible patients in Oregon, Washington, and Arizona, subject to prescribing requirements and the dispensing pharmacy’s ability to ship to the delivery address.
Orders are processed after clinical approval and completion of any required prescription and payment steps. Processing and delivery times vary by pharmacy, medication availability, and carrier. Any applicable order cutoff or estimated delivery timeframe will be provided before purchase. Delivery estimates are not guarantees.
Once medication has been dispensed, we do not accept returns or provide refunds for changes of mind, cancellation, or a decision not to use the medication, except as required by law.
If your shipment is missing, damaged, incorrect, or appears to have been stored or transported improperly, contact TeleHealthNP promptly. We will coordinate with the dispensing pharmacy and carrier to investigate and determine the appropriate resolution, which may include replacement or a refund where warranted or required by law. Do not use medication that appears damaged or incorrect until you receive guidance from the dispensing pharmacy or your clinician.
Cancellation of a subscription applies to future orders according to the cancellation terms below and does not reverse an order already dispensed.
6. Prices, Included Services, and Payment
The price and inclusions for your chosen service must be displayed before purchase. Review whether medication, shipping, testing, follow-ups, and other charges are included. A lower-cost care plan does not necessarily include pharmacy charges. We will not treat a website description as permission to impose an undisclosed fee.
Payments are processed through Stripe and any disclosed connected checkout system. You authorize the charge you accept at checkout. Recurring charges require the separate recurring-payment consent described below. Keep billing details current and contact us about suspected errors or unauthorized charges. Nothing here limits a lawful billing dispute or chargeback right.
We do not guarantee insurance reimbursement or HSA/FSA eligibility. Any legally required good-faith estimate or billing notice is provided separately; these Terms do not replace it.
If a payment fails, we may contact you to resolve it. Any service restriction must comply with continuity-of-care requirements and cannot eliminate access rights to medical records.
7. Subscription Billing, Automatic Renewal, and Cancellation
If you choose a recurring plan, checkout must clearly disclose the amount, billing frequency, first charge date, renewal terms, minimum commitment if any, and the cancellation method before obtaining your express consent. A calendar-month plan and an every-four-weeks plan are different; the accepted checkout terms must specify which applies.
Your first subscription payment is charged at signup. If you are not clinically approved for treatment under your selected plan, we will refund that payment and cancel the subscription so no further renewal charges occur. Payment does not guarantee a prescription.
By enrolling and authorizing recurring payments, you agree that your subscription will automatically renew every four weeks until canceled. Every four weeks means every 28 days, not once per calendar month.
Subscription prices are:
Flex Weight Loss: $139 every four weeks.
Semaglutide Complete: $299 every four weeks.
Tirzepatide Premium: $399 every four weeks.
Semaglutide Complete and Tirzepatide Premium include the clinical services, medication, and standard shipping described for the selected plan. Flex Weight Loss includes the plan’s clinical services and included compounded medications shipped to you. Brand-name medications filled at a pharmacy are not included in the Flex subscription price; you are responsible for paying the pharmacy separately at pickup. All prescriptions remain subject to clinical approval.
There is no minimum subscription commitment. To prevent your next renewal charge, cancel at least 24 hours before the scheduled charge using any of these methods:

For SMS or portal requests, the time we receive your cancellation request determines whether you met the deadline. Requests received less than 24 hours before the scheduled charge may take effect after that renewal.
Cancellation stops future renewals. You retain access to the care included in your plan through the end of your paid subscription period. Cancellation alone does not result in a prorated refund for the unused portion of that period, except as required by law.
Once a medication order has been submitted for fulfillment, we generally do not accept cancellation of that order, even if it has not yet shipped. We will correct orders placed because of our error and provide any remedy required by law. Medication that has been dispensed cannot be returned or refunded, except as required by law. Missing, damaged, or incorrect shipments are handled under our Medication Shipping, Returns, and Refunds policy.
We will provide advance notice of changes to subscription pricing and obtain any consent required by applicable law.
Cancel by logging into your account and canceling your subscription, or by contacting 602-810-0868. We will explain or confirm the effective date and any already-incurred charges. Canceling recurring billing stops future renewals as described in the accepted terms; it does not by itself decide the clinical steps needed to end or transition treatment. Do not discontinue medication abruptly without appropriate clinical advice.
Pause policy: Pausing Your Subscription
You may request a subscription pause by contacting TeleHealthNP through SMS or your patient portal and explaining the reason for your request. We will confirm the pause’s effective date and any arrangements for resuming treatment.
To prevent your next renewal charge, request a pause at least 24 hours before the scheduled charge. During a confirmed pause, recurring subscription charges and new medication orders are suspended. A pause does not reverse charges already processed or cancel medication orders already submitted for fulfillment; those remain subject to our cancellation and refund policies.
Before restarting medication, contact your treating nurse practitioner to confirm an appropriate restart plan. Your nurse practitioner will consider the medication, your previous dose, the length of the interruption, side effects, and any changes in your health. An updated evaluation may be required.
An interruption longer than four weeks, particularly if you were taking a higher dose, may require restarting at a lower dose at your nurse practitioner’s discretion. A shorter interruption may also require a dose adjustment. Do not resume your previous dose without your clinician’s guidance.
We will confirm the restart date and applicable subscription charge with you before recurring billing and medication orders resume.
We will give required advance notice of price or renewal changes and obtain consent where required. Material changes do not authorize retroactive charges. We will make cancellation available as required by applicable law and will not condition it on agreeing to new marketing or privacy terms.
8. Refunds, Missed Appointments, and Promotions
Refunds and Practice-Canceled Services: Consultations and appointments do not carry a separate fee.
If you pay at signup but are not clinically approved for treatment under your selected plan, we will refund your initial subscription payment and cancel future renewals.
If TeleHealthNP cancels your care or cannot provide the subscribed service, we will cancel your subscription and stop future charges. If payment has already been collected for the affected subscription period, we will refund that payment. This exception applies to practice-initiated cancellations and does not change the separate rules for patient-requested cancellations.
When you cancel your subscription, you retain access to included care through the end of your paid period. We do not provide prorated refunds solely because you cancel or do not use all available services, except as required by law. Cancellation requests remain subject to the subscription cancellation deadline.
Once a medication order has been submitted for fulfillment, we generally do not accept cancellation or provide a refund for a change of mind, even if the medication has not yet shipped. Dispensed medication cannot be returned or refunded, except as required by law.
If you believe you were charged twice, charged incorrectly, or charged after a timely cancellation request, contact us by SMS or through your patient portal. We will investigate and refund any verified erroneous charge.
For missing, damaged, incorrect, or potentially compromised medication, contact us promptly. We will coordinate with the dispensing pharmacy and carrier to determine the appropriate resolution, including replacement or a refund where warranted or required by law. Do not use medication that appears damaged or incorrect until you receive guidance from the pharmacy or your clinician.
Nothing in this policy limits rights or remedies that cannot be waived under applicable law.
Rescheduling and Missed Appointments: Please cancel or reschedule appointments at least 24 hours in advance whenever possible. You may reschedule directly through Acuity using your appointment confirmation link, or contact TeleHealthNP by SMS or through your patient portal for assistance.
If you cancel or miss an appointment, we will make reasonable efforts to help you reschedule, subject to availability. We do not charge appointment cancellation or missed-appointment fees.
Canceling or rescheduling an appointment does not cancel or pause your subscription. To stop future subscription charges, follow the subscription cancellation or pause procedures described in these Terms.
Promotions are governed by the terms disclosed with the offer, including eligibility, expiration, exclusions, and whether a discount applies only to the first payment. A promotional price does not change later renewal pricing unless expressly stated.
9. Technology, Accounts, and Communications
We use Squarespace for the website, Acuity for scheduling, SimplePractice for electronic records and designated patient functions, Stripe for payments, Gmail for email, and AT&T telecommunications for telephone and text communications. Providers may supply separate platform terms. Our use of these tools does not promise that every feature is suitable for clinical information.
Protect your account credentials, do not share access improperly, and notify us of suspected unauthorized use. You must not impersonate another person, submit intentionally false information, attempt unauthorized access, disrupt the systems, or threaten staff or other users. A lawful complaint, critical review, or exercise of patient rights is not prohibited conduct.
Use the clinical communication method designated by your care team for detailed medical information. Do not send medical records through public website forms or payment fields. Email and standard SMS have privacy risks. Communication preferences, marketing choices, and data practices are addressed in our privacy notices; accepting these Terms does not provide a blanket privacy authorization.
Typical response times and staffed hours: Availability and Response Times
TeleHealthNP does not maintain fixed office hours. Our care team is generally available between 8:00 a.m. and 7:00 p.m. Pacific Time on most days, but availability varies, and immediate responses are not guaranteed. Please allow 24–48 hours for a response to routine messages.
SMS, email, and patient portal messages are not continuously monitored and should not be used for emergencies or concerns requiring immediate attention. For a medical emergency, call 911 or go to the nearest emergency department.
Direct access to your clinician does not mean continuous availability or immediate response. If we use an automated text program, its consent and opt-out terms will be presented separately. Message and data rates may apply.
10. Privacy and Records
Our Privacy Policy, Notice of Privacy Practices, and Washington Consumer Health Data Privacy Policy explain the information practices and rights within their respective scopes. They are notices, not waivers of rights. These Terms do not authorize sale, advertising disclosure, or other use of health information that requires separate consent or authorization.

Canceling a plan or closing an account does not automatically delete medical records that must be retained. Contact our privacy contact to exercise applicable access, amendment, deletion, or other rights.
11. Website Materials and Patient Submissions
TeleHealthNP or its licensors own the website materials except where otherwise stated. You may access them for personal, lawful use. Do not copy or commercially redistribute protected materials without permission, except as law permits. This clause does not restrict lawful quotation, accessibility tools, or your use of your own records.
You retain your rights in information you submit. We may use submissions as necessary to respond, provide the requested services, maintain required records, and exercise other lawful purposes described in our privacy notices. These Terms do not grant a general license to publish your health information, likeness, or testimonial.
12. Availability, Suspension, and Ending Care
We aim to keep the website and service systems available, but outages and interruptions can occur. Contact the practice for an alternative when a system problem affects your care, scheduling, or billing.
We may restrict website or account access for a material violation, security concern, or legal requirement, with notice when appropriate. Ending an established treatment relationship requires compliance with applicable professional duties, including appropriate notice, transition support, and records access. Account suspension does not authorize patient abandonment or forfeiture of legal rights.
13. Legal Rights and Disputes
These Terms do not waive professional standards of care, informed-consent obligations, privacy rights, consumer protections, or liability that cannot lawfully be excluded. We do not promise uninterrupted technology or a particular clinical result, but remain responsible for our duties under law. A third party's involvement does not automatically eliminate our own responsibilities.
If a concern arises, contact us so we can try to resolve it. This informal option does not delay a legal deadline or prevent you from contacting a regulator, disputing a charge, or seeking available legal remedies. These Terms do not impose mandatory arbitration, a class-action waiver, or a shortened claim deadline.
Applicable Law and Jurisdiction: These Terms and any disputes relating to our services are subject to applicable federal and state law, including mandatory protections applicable to the patient. Any court proceeding must be brought in a court with jurisdiction over the parties and the dispute, in a location permitted by applicable law.
Nothing in these Terms limits patient rights, consumer protections, or remedies that cannot legally be waived.
If a provision is unenforceable, the remaining provisions apply to the extent lawful. A failure to enforce a provision on one occasion does not necessarily waive it on another.
14. Changes and Contact
We may revise these Terms prospectively and will identify the revised effective date. We will provide notice and obtain renewed assent when required, including for material changes affecting an ongoing service or recurring charge. Changes do not retroactively alter completed purchases or authorize an undisclosed use of health information.
